End of life planning

This link to NHS Choices website contains information about what someone can expect during end of life care and the things they may want to think about. These include how and where they want to be cared for, as well as financial issues.

Letting family know about a person’s wishes could help them if they ever have to make decisions about the person’s care.  Knowing that they are acting in accordance with the person’s wishes can remove some of the stress from a very difficult situation.
