Once you have helped an individual to set their goals within the context of what’s really important to them, and developed a shared understanding of what is helping them and what is getting in the way (or missing), then it is time to agree the action plan or “to do list”.
“How could I change this to achieve my goal?” encourages the individual to reflect on what is helping them and what is getting in the way, and then, with the potential help of care professionals, carers, friends and family members to
generate a set of opportunities for improvement that support the overall achievement of the goal and the best possible quality of life for the older person living with frailty.
- Support the overall achievement of the goal
- Reflect what is really important to the individual and their own assets / areas of resilience
- Address what is missing or getting in the way as far as possible
