Effectiveness Matters: Recognising and Managing Frailty in Primary Care is a high level synthesis of high quality evidence aligned to frailty recognition and management.

It draws on the recommendations for screening for frailty within the BGS Fit for Frailty toolkit and has been updated to reflect the recent NICE Guidance NG56: Multimorbidity guideline.

A useful summary of what interventions have been shown to improve outcomes for people living with frailty is also provided.  This includes sections on comprehensive geriatric assessment, exercise, medication review, supported self-management, case management and advance care planning.

It also signposts to guidance and resources to support the GP core contract (2017-18) regarding frailty that are available on the NHS England website and can be accessed here.
