Understanding what is important to the older person with frailty is at the heart of a person centred approach to supported self-care. Research indicates five broad themes of what may be important, with each one needing to be explored and interpreted as it relates to the individual and their situation at that point in time. This question should be revisited regularly throughout the individual’s journey of frailty because evidence shows that a person’s answer may change along the way, as their condition progresses or their personal priorities change.
