The role of the Elderly Care Facilitator has been crucial to the success of this project. The Elderly Care Facilitator assesses needs, communicates and educates, and provides an ‘accessible bridge’ between older people, their carers, health professionals, social support groups and services, voluntary groups and a whole range of statutory services. These roles have been nurtured and developed through the course of the project with a local lead coordinator and lead nurse practitioner based at Madeley Practice available to provide initial training, supervision and support to the other practices involved across the locality.
